What “% w/v” means — and what it costs to get it wrong

Correct — made up TO 100 ml. Powder first, then water until the total volume reads 100 ml. 18 g + 87 ml water → 100 ml at 18.00% w/v. Both source documents use this reading: Nutricia states “13.5 g + 90 ml water = 100 ml feed”; the dietetic SOP states “20.5 g formula + water up to 100 ml (powder first)”.
Incorrect — added INTO 100 ml of water. Powder added to a measured 100 ml of water. 18 g + 100 ml water → 112.96 ml at 15.93% w/v — an 11.5% under-concentration that does not look wrong on the page.
